This 1099 web packet has been assembled by the Human Resources, and Risk Management Departments to assist you in completing the process of becoming a 1099 employee of HIES Inc.

The items are informative and help answer some of the questions you will have regarding HIES Inc. 1099 employment. As a new employee you will need to fill out, read and sign a lot of documentation. This web page is to help guide you in completing this process. Below are links to several pdf forms that you may fill out online then save to your computer and email to the human resources department. These documents must be signed. You can either print them out after you type in the fields sign them with your pen scan them and email them to human resorces or you may sign the PDF on the form itself (More information on how to digitally sign these documents)

Employee Handbook


First, it is essential that you read the entire handbook fill out and sign the documentation within. You should have been given a handbook upon your acceptance of an employment offer within HIES.

Sexual Harassment Policy


Second, it is essential that you read the entire sexual harassment policy, fill out and sign the documentation within. You should have been given a sexual harassment policy guidebook upon your acceptance of an employment offer within HIES.

Within this guidebook there are certain pages that must be filled out and completed after you have read the above guidebook please fill out the pages below in regards to the Sexual Harassment Policy:

General Documentation:


  1. 1099 Agreement, Employment Agreement & Offer Letter: You will need to attached these items to your packet when you email and mail all the items.
  2. 1099 Employee information sheet
  3. W-9 Form: To withhold the correct federal income tax from your pay. Consider completing a new Form W-4 each year and when your personal or financial situation changes.
  4. Military Base Pass Information Form
  5. Criminal History Check
  6. Business card request form
  7. Logo Apparel Request Form: You will be able to order 4 logo shirts, one hard hat, and one saftey vest for your probationary period. You will need these items to complete your work.
  8. Request For Email Form
  9. You will need to have hard toed boots for your first day on the job. Keep your reciept and after your probationary period is over and you are hired full time you will be reimbursed up to $70.
  10. You will need to attach a copy of your U.S. driver’s license;
State-issued non-driver identity card or U.S. passport.
  11. During your career at HIES Inc. there will be times when you will have a scheduled Dr. appointment, want to take a personal day or vacation, have jury duty or other. Before you plan on taking days off you will be required to fill out a form to have management approve your leave. You may review download this file on the website under the employee resources webpage. Please take a look at the form and get aquainted. We understand that there are a lot of questions that arise from PTO (paid time off) and how it is managed and we would like to clear up as many questions as you have before you need to take time off.   12. While working for HIES Inc. it is critical that we log our hours we work for each project in which we service. You will be able to access this online time card through the website on the employee resources page. You will need to log in and complete your timecard everyday. You can view the time sheet login webpage by going to the HIES Inc. website and in the footer there is a link to "employee resources" about the middle of the page there is a link that states timesheet. Or you may access this timesheet by clicking on this link and saving it in your browser You will be issued a username & password when your files are added to our system.
  13. You will need to sign your name on a blank piece of paper. This signature will be created into a signature with the HIES Inc.watermark. These are for transmitting digital signatures throughout the company. This is NOT for the use of HIES to sign your name it will be used for yourself to sign your name on applicable documentation.

  4. Contact the office and make an appointment for your physical

Submittal:
  1. Make sure that you have downloaded the "Starter Guide Check List" and have completed all items. Place this sheet on the top of the stack of information. Follow the items in the stack as they are presented in the "starter guide check list" they must be in order for processing.
  2. Email all forms in jpg or pdf form to the Human Resource department
In the subject line place: New Employee: State location of your main office (HI or Guam) and insert your first and last name.
  3. Mail all originally signed forms to the main office for processing:
